Volume loss in the face does not happen all at once, and it does not happen everywhere equally. It follows the anatomy of the facial fat compartments — the discrete pockets of fat that give the youthful face its three-dimensional lifted quality — as they gradually deflate and descend through the 30s, 40s, and beyond. The temporal fat pad thins first, creating a subtle hollowing of the upper face. The malar and submalar fat compartments lose projection and descend, creating the paradox of midface hollowness alongside lower face heaviness as the same fat that once filled the cheek now sits at and below the jawline. A firm, highly cross-linked, cohesive gel is appropriate for structural support at the cheekbone level. A soft, pliable, less cross-linked formulation is appropriate for the lip. What We Treat and How
